At TIDE Connect, our vision is a more equitable healthcare system where every person can access the care, coordination and practical support they need to remain safe, independent and connected in their own home for longer.
We are a family-owned and operated, locally based provider supporting NDIS participants through support coordination, trauma-informed care, and flexible service delivery, both in-person and via telehealth.
Our mission is to deliver person-centred support coordination that strengthens choice and control, connects people with appropriate services, and helps them achieve their goals while remaining independent and engaged in their communit
